Please fill out a registration form for each child, and provide a copy of the child's birth certificate along with the Medical Release and a signed Parental Code of Conduct. The packet also includes a volunteer form for parents interested in Coaching or helping out! These forms need to be turned in at Town Hall by September 2nd with the $10/player fee. There is a late registration fee of $25/player if paid after September 2nd.
